Question about radio jingles and ASCAP ...

In addition to the license fee for the piece of music accompanying a voice over in a radio ad, is the music entitled to a royalty if it's registered with ASCAP?

If so, does the song's info need to be submitted to the radio stations that play the ad in order for ASCAP to keep track?

Where can I find a fairly standard license agreement for me and the person paying for the ad?

This is my first attempt at this, so please bear with me ...
