Studio Musician Release Forms?

Does anyone have, or know where I can get, a release form for the musicians who participated in the recording of my tracks? I've Googled, but not found anything that really resembles what I think I need. The musicians were all paid for their performances, and releases are required for some of the listings here, in the event of a forward.Thanks,Jeff
