Using Personal or Artist name on contract?

Good Day All.

Is there a distinction between placing your Personal vs Artist name in the Name/Composer slots on a contract? With my PROs I am registered as Devine Score Music if this matters.
For example on the first page they have Name: in with the address section, then on the last page Composer:

Appreciate any feedback.

Caveat: I'm not a music lawyer.
If you've registered 'Devine Score' as a DBA name, I'd think the choice would be open: how do you want to be known to the library? Either way would be legally binding.

I am not an entertainment lawyer but if you are registered under Devine Score Music with your PRO it seems to me that would be the name you are doing business as; however you are the contact person for that entity so your name and address and phone etc would be important. To make sure you do it correctly for the org you are signing with, why not ask them how they would like it completed.
