ProTools & MIDI books?

Hi,
As a former recording engineer from the days of 2" analog, I'm using Pro Tools now & then for my own projects and often run into questions that the books I have either don't answer clearly or don't index in an obvious way. Other than the Digi publications and Gina Fant-Saez's "Pro Tools for Musicians & Songwriters" does anyone have any recommendations for a thorough, straightforward, meticulously-indexed guide?
I'm considering John Keane's "Musician's Guide to Pro Tools." Any opinions on that one?
I'm also starting to deal with MIDI for the first time in my life. Any thorough, clearly written books to recommend on that subject? It needs to have beginner basics as well as detailed instructions for all possible situations (and a really comprehensive index!).
