Meta Data for a Cover Song

Ok so I need help with entering the metadata of one of the cover songs I re-arranged and had a collaborator with me.
Here's my proposed list, if you could let me know if it's ok or not..

Track Title: Original Track Tile ft. My collaborator (a singer)
Track Artist: Myself? My collaborator and I?
Composer: The original composer(s), or myself?
Year: 2017 (the original was released in 2013)
Contact Info (In the comments section): My web address and email

Do I need to include the original publisher?
Do I need to include somewhere that the singer and I collaborated 50/50?
Do I need to include my collaborator PRO IPI#? (I'm including mine)

Is there anything else I should be aware of?
